550995 Lessons Learned from Analyzing an Explosion Accident at Shanghai Secco Chemical Plant

On May 12, 2018, six workers from Shanghai Aijinke Engineering Construction Services Co., Ltd. died on the spot of an explosion when they were maintaining a benzene tank in the tank farm at Shanghai Secco Petrochemical Co., Ltd. The accident was characterized as a disastrous accident caused by hot work in a confined space, which resulted in massive casualties and a direct economic loss of 11.66 million yuan. This paper aimed to analyze the accident occurrence process in a chronological order firstly by using the event sequence diagram (ESD). Multilayer Bow-tie Analysis was adopted to explore and classify the causes of the explosion of the benzene tank, along with the corresponding consequences, and the preventive measures. The analyses show that the primary inducements were the unsafe behavior of on-site workers, hazardous conditions of in-tank substances, risky factors in the construction environment, and deficiencies in management. Casualties, fire and explosion, equipment damage, and property losses were all included as consequences. The FLACS software was used to simulate the variation of explosion pressure and explosion temperature varying with time when benzene vapor was ignited at the tank bottom during the accident, and a flow field diagram for the explosion was achieved correspondingly. The results obtained are of directive significance for investigation of similar accidents in the future and are instructive to equipment safety evaluation as well. Based on the qualitative and quantitative analyses, we emphasize two considerable reasons leading to the accident, which includes inapposite flammable gases detection points before the hot work and failing to test the flammable gases during the hot work. Finally, the paper concludes with a summary of lessons learned from the accident as well as suggestions for accident prevention and safety improvement.
