Description:
The likelihood of initiating a hazardous event is much hirer during upset conditions or abnormal, non-routine periods of operation. These are also the operating periods where safety functions are routinely disabled following an extended turnaround. This session is intended to draw papers from organizations that have implemented written management systems, methods, or tools to identify, prepare for and manage abnormal events.
