Industry data has shown that fatality rate does not necessarily correlate with traditional safety metrics, such as incident rates or injury frequencies. Even organizations with world class safety performance can exhibit fatality trend that is not supported by their otherwise good safety records. This presentation proposes that the application of risk-based approach to daily tasks and conditions can help prevent fatalities. The idea hinges on equipping personnel with the necessary tools to recognize situations with high potential for serious injuries or fatalities, and ensuring that all protection mechanisms are in place and will function as intended when required.
