Process Safety Management (PSM) was developed many years ago to reduce hazards and risk for Oil Industry. In the last 3 years serious accidents had happened in the Oil Industry that may require us to re-think about the PSM programs.

It has been seen in many HAZOP studies, that HAZOP -part of PSM program- does not reflect a real commitment to safety; but the “need” to meet regulations.

This papers initiative is to call organizations/companies to think and be safe instead of fulfill project or government rules.

Full involvement of the organisation, starting with managers is required for the PSM to work; in this paper many practical examples are given on how this program was implemented several years ago and why it was considered very successful.
