Many incidents have been triggered by unanticipated interactions with utility systems. Utility systems have many connections to processes, both physical and electrical. This interconnectivity results in common mode failures and the potential for cross-contamination. This paper will use “real-plant” incidents to illustrate what can happen when the hazards of interactions are not identified or understood. Common failures which occur in utility systems will also be discussed.
